The Membership Document — historically Form #1726, often referenced today as Form #100 — is how you turn a prospect into a Knight or move an existing Knight between councils. Unlike the calendar-driven compliance forms (#185, #365, #1728, #1295), this one runs year-round — you file it whenever you have a new candidate or a transferring member.
Heads-up on the form number. Supreme has been consolidating membership paperwork in recent years, and the printable PDF is not currently listed on kofc.org's officer forms page. Most councils now submit membership proposals through Member Management (the online officer portal) rather than the paper #1726. The process below is unchanged; pull the current form or digital flow from Supreme's portal when you onboard a real candidate.
Two distinct uses
Same form family, different sections, different signatories.
The path from prospect to Knight. Candidate fills out the form, a sponsor endorses, the council votes, and the Exemplification confers the degrees.
Moving an existing Knight from one council to another. No re-exemplification — once a Knight, always a Knight.
New member proposal flow
The candidate is a practical Catholic man, 18 or older, in good standing with the Church. Usually introduced by an existing member, sometimes by the parish priest.
Candidate provides personal information — name, date of birth, address, parish, sponsor (the existing Knight who's proposing them). Candidate signs.
Council reads the proposal at a business meeting. Any objections handled per bylaws. Majority vote typically accepts the candidate.
Under the current Exemplification (since the 2020 reform), all three degrees can be conferred in a single ceremony. Candidate participates and formally becomes a Knight.
Completed form — GK signature, candidate signature, degree completion noted — goes to Supreme. Supreme issues the membership number and card.
Membership Director introduces the new Knight at the next business meeting. FS enters the new member into the roster. Insurance Field Agent is notified for the insurance-eligibility conversation.
Transfer flow
Transfers happen when a Knight moves to a new city, joins a parish with its own council, or was inactive in one council and is restarting in another.
Typically by contacting the receiving council's FS or Membership Director.
Same process as a new-member vote.
Different checkbox / section than new member.
Signals council acceptance.
Supreme notifies the original council; member is dropped from its roster and added to the receiving council.
Transfers don't require re-doing the Admission/Exemplification. Once a Knight, always a Knight — membership is Order-wide, not council-specific.

Common mistakes
Wrong date of birth, missing parish, wrong address — these slow Supreme processing by weeks.
Every new member needs a sponsor. If a candidate walked in without one, the Membership Director becomes the default sponsor.
#1726 isn't complete until the Exemplification is recorded on it.
Plenty of councils conduct the Exemplification and then the paperwork sits on a desk for a month. The candidate isn't officially a Knight until Supreme processes.
Most of the time Supreme handles the notification, but if the member's original council is active, a courtesy note to their FS smooths the transition.
What happens after Supreme processes
New members:
- Supreme issues the membership number (typically within 2–4 weeks).
- Membership card mailed to the member.
- Member is added to Supreme records and becomes eligible for Per Capita counting on the next calendar cycle.
- Council receives confirmation and updates its internal roster.
Transfers:
- Supreme moves the member from the original council to the receiving council.
- Original council's roster decreases by 1; receiving council's increases by 1.
- Per Capita for the member follows the receiving council from the next billing cycle.
One more thing
#1726 is the form that directly grows (or shrinks) your council. Every Father McGivney Award win is built on signed #1726s. Every member who walks away without officially resigning is a candidate for a courtesy call before they transfer or go inactive.
Process new members fast. Process transfers cleanly. Update the roster in real time. That's the retention play on this form.
