When to use it
Use this to replace paper Form 1728A worksheets at year-end. Open a survey year, members submit their hours through a personalized link, and DuesSync totals Section III across every submission. Form 1728 reports the prior calendar year's activity and is due January 31, so you'll typically open a survey late in the reporting year and close it in January.
Step by step
- Confirm the feature is enabled. It's a premium add-on controlled by DuesSync admin — check Configurations → Fraternal Survey Setup (
/dashboard/configurations/survey). If it shows "Not enabled," email admin@duessync.com with your council number. - Go to Compliance → Fraternal Activity.
- Click + New Survey Year. Set the Survey Year (the calendar year the activity covers) and the Expiry Date (defaults to Jan 31 of the following year — the Form 1728 due date). Click Create.
- Open the survey from the active-survey card via Manage Survey → (or click the year in the list) to send links, track responses, and enter Section I (meetings) and Section II (charitable disbursements).
- Watch the response ring and the "submitted / in progress / not started" counts to see who still needs to submit.
- When responses are in, review the auto-aggregated Section III totals and submit to Supreme; the year's status moves to Submitted to Supreme.
Tips & gotchas
- Form 1728 due date is January 31 for the prior calendar year — don't set the expiry to Jan 31 of the reporting year itself (that's already in the past).
- Survey years are filtered to the active Columbian Year context; use the CY pill if you don't see the year you expect.
- The per-capita Obligations page (
/dashboard/compliance/obligations) is a separate surface — it covers Supreme/State/District assessments, not Form 1728. - Assembly Survey (Form 1728C) and year-round logging are marked "Coming Soon."
