Service Program Report (Form 365)

Records your council's Program, Community, and Family directors so Supreme can route Faith in Action resources, and produces/submits Form 365.

Who uses it
Financial Secretary (assigns directors and submits); the Grand Knight co-signs the printed form.
Where

When to use it

File Form 365, the Service Program Personnel Report, near the start of each Columbian Year — submit by July 1. Assign your directors as soon as the new term's appointments are made so director welcome emails go out and so Supreme can reach the right people. Form 365 is also a Star Council prerequisite, so keep it filed and current.

Step by step

  1. Go to Programs → Program Directors. Each program role (Program Director, Community Director, Family Director, and the others on the printed form) shows its current assignee or "Unassigned."
  2. Click into a role to open the search box, type a name or member number, and select the member to assign that director.
  3. Use the remove/clear control on a row to vacate a role; reopen the search to reassign.
  4. Use the PDF preview button to review the generated Form 365 before sending.
  5. Use Submit to Supreme to transmit Form 365 (you can also hit Submit on the Compliance hub's Form 365 card). Optionally notify the directors and Deputy District Deputy.
  6. To mark the form filed when you submitted out-of-band, open the printable Form 365 manager (via the compliance year) and use Mark Submitted.

Tips & gotchas

  • Assigning a director's email here is what kicks off Safe Environment credentials — Praesidium emails each officer a training link, so accurate director emails matter.
  • The Compliance hub counts "directors assigned" (e.g., 3 of 4); a partly-filled roster still shows as Pending until submitted.
  • The printable Form 365 includes Financial Secretary and Grand Knight signature lines — both sign the paper copy.
  • The director list mirrors the current printed KoC form; roles dropped from that form aren't shown here even if older records exist.
