Every KoC Council Form — The Complete List, Explained

Why this page exists

Every Knights of Columbus council runs on a set of standard Supreme forms. New FSs and GKs often meet these one at a time, in whatever order the calendar throws them — and spend their first year just figuring out which form is which.

This page lists every form a council-level officer is likely to touch, what each does, who prepares it, who signs it, and when it's due. Use it as a map, not a substitute for individual form guides.


The compliance forms (Good Standing)

These five are the ones that determine your council's Good Standing status. Miss any one and you're out of Good Standing until it's filed.

Form #185 — Report of Officers Chosen for the Term

  • What it does: Reports your newly elected officers to Supreme for the new fraternal year.
  • Prepared by: Financial Secretary (data from the election results)
  • Signed by: Grand Knight (incoming)
  • Due: June 30
  • Covers: The incoming fraternal year's elected officer slate (GK, DGK, Chancellor, Recorder, Advocate, Treasurer, Warden, Inside Guard, Outside Guard, and three Trustees).
  • Does not cover: The Chaplain (not an elected officer) or the Financial Secretary (appointed). Those go on Form #365.

Form #365 — Service Program Personnel Report

  • What it does: Reports your program team (pillar directors, Membership Director, Retention Chairman, Chaplain, etc.) and starts your Columbian Award clock.
  • Prepared by: Financial Secretary (data from the Deputy Grand Knight / Program Director)
  • Signed by: Grand Knight
  • Due: July 1
  • Covers: Up to 18 program-role slots. Not all must be filled.
  • Why it matters: Without #365 on file, no Columbian Award credits count.

Form #1728 — Annual Survey of Fraternal Activity

  • What it does: Reports your council's program activity and member charitable hours to Supreme. Feeds into the Order's IRS 501(c)(8) reporting.
  • Prepared by: Financial Secretary (in consultation with Program Director)
  • Signed by: Grand Knight + Financial Secretary
  • Due: January 31 — covers the prior calendar year (January 1 – December 31), not the fraternal year
  • Why it matters: Required for Good Standing. The only major form that runs on the calendar year instead of the fraternal year.

Form #1295 — Annual Council Audit

  • What it does: Reports the Trustees' audit of the council's financial records for the fraternal year just ended.
  • Prepared by: Trustees (with Financial Secretary providing records)
  • Signed by: All three Trustees + Grand Knight
  • Due: August 15, covering the prior fraternal year (July 1 – June 30).
  • Why it matters: Required for Good Standing. Late filing pauses Star Council eligibility.

Per Capita Assessments

  • What they do: Supreme bills councils three per-member assessments together, twice a year: Per Capita Tax ($1.75/levy), Catholic Advertising ($0.50/levy), and Culture of Life ($1.00/levy). Combined: $3.25/member/levy, $6.50/member/year.
  • Prepared by: Financial Secretary
  • Paid by: Financial Secretary (from the council's account)
  • Due: Levy 1 on January 1 (pay by April 10); Levy 2 on July 1 (pay by October 10). State Per Capita is separate — rate and date both vary by state.
  • Roster basis: Total membership including inactive and honorary members. Excludes honorary life and disabled members.
  • Why it matters: Non-payment is the fastest way to lose Good Standing.

The award forms

Form SP-7 — Columbian Award Application

  • What it does: Submits your council's year of program activity for the Columbian Award (the programs leg of Star Council).
  • Prepared by: Program Director (usually the DGK)
  • Signed by: Program Director + Grand Knight
  • Due: June 30
  • Covers: All credit-eligible programs across the four Faith in Action pillars.
  • Target: 16 total credits, minimum 4 per pillar.
  • Why it matters: Required for Columbian Award and therefore Star Council.

The administrative forms

Form #1726 — New Member Proposal Form / Member Transfer

  • What it does: Submits a new member candidate for processing (or transfers an existing member between councils).
  • Prepared by: Membership Director (new proposals) or Financial Secretary (transfers)
  • Signed by: Grand Knight + candidate
  • Due: Submitted to Supreme upon completion (not a calendar-driven form)
  • Why it matters: Member isn't officially a Knight until #1726 is processed by Supreme.

Voucher / Form 157 — Council Expenditure Authorization

  • What it does: Authorizes a council expenditure. Paper or digital.
  • Prepared by: Financial Secretary (draws up the voucher based on an approved expense)
  • Signed by: Grand Knight (approves) → Treasurer (pays)
  • Due: As needed throughout the year
  • Why it matters: No council funds are supposed to leave the account without a voucher. This is the audit trail.

The insurance track — Fraternal Benefit Events

Founders' Award eligibility

  • What it does: Councils earn the Founders' Award by hosting or promoting Fraternal Benefit Events (FBEs) during the fraternal year. Supreme determines eligibility automatically — there is no separate "Founders' Award application form" to submit.
  • Requirement (per Fraternal Excellence Guide 2025): at least two FBEs during the fraternal year (College and Military councils: one). Combined attendance must reach 10 (councils ≤ 99 members) or 14 (100+ members). If the GK and DGK each have a one-on-one fraternal-benefits review with the Field Agent, those two meetings count as one of the required FBEs (not available to College/Military councils).
  • Who runs them: the Order's General Agent and Field Agent are the primary presenters. They register the events upstream with Supreme, which is what makes the council's participation count.
  • Council's job: coordinate with your Field Agent early in the fraternal year so the two events are scheduled and promoted. Track attendance.
  • Due: Events must occur within the fraternal year (July 1 – June 30).
  • Why it matters: Required for Founders' Award and therefore Star Council.
